The medical examination process should be completed before the visa appointment. The medical examination and laboratory tests may only be done at the addresses given below. Each applicant is required to show his/her passport as identification at each step of the medical examination process. If you have the visa case number, please bring it, but it is not required for the medical appointment. Medical examinations from physicians or clinics other than those listed below, even those from the United States, are not accepted. The per-person fee for a physical exam in Poland is 200,- and additional 25,- zloty for completing the vaccination report. The fee, which has been established in agreement with the American Embassy in Warsaw, is paid directly to the physician. An additional fee is charged for every vaccination. Different rates are charged in Belarus, Latvia, Lithuania and Ukraine. Prices of the laboratory tests and medical examinations are subject to change. Maximum validity of medical examination results is one year (with at least 6 months’ validity required on the day of visa issuance). If the exam has less than 6 months’ validity, the visa is limited to the life of the exam. The vaccination report is not required for K -fiance(e)s and K3/K4 applicants. Immigrant visa applicants, however, must submit this report as part of the examination process.
Visa applicants younger than 15 do not need chest X-ray or blood tests, although they must undergo a general medical examination. Applicants 15 or older should bring the chest X-ray and the results of the blood tests with them to a panel physician shown below. All applicants 15 years and older must be screened for syphilis and HIV infection (HIV-1/HIV-2 antibodies). Applicants below 16 years of age should bring their Child's Health Book. All visa applicants must bring one color photograph (in addition to two photographs required for their visa application) and their passport to the physical exam. Children under 14 should be accompanied by at least one parent or a guardian.
Before leaving the X-ray lab, make sure that the following information is imprinted on the X-ray itself: your first and last name, passport number, date the X-ray was taken and the name of the X-ray lab. Pregnant visa applicants do not provide a chest X-ray. Immunizations for pregnant women are done only at their wish and upon the recommendation of the panel physician -- never using “live vaccine.”
Take the chest X-ray to the United States. Do not bring it to the Embassy.
No appointment is required for the chest X-ray or blood tests. Nevertheless you must make an appointment with a panel physician for your general medical examination.
WARSZAWA
X-ray

Syphilis and HIV Blood Screening – Fasting not required

Blood samples are taken every day except Saturdays and holidays between 7:00-14:30. Results of samples taken between 7:00-8:30 are ready to be picked up the same day between 12:00-14:15; those taken between 8:30-12:00 between 14:00-14:15; and those taken after 12:00 the next day between 12:00-14:15. Results can also be picked up any other day between 7:00-14:15 except Saturdays and holidays. The charge of 46,- zloty for the blood tests is payable directly to the Blood Center.

Blood samples are taken every day except Saturdays and holidays between 8:00-14:00. Results of samples taken between 8:00-10:00 are ready the same day between 13:00-14:30; those taken after 10:00 are ready the next day between 13:00 - 14:30 or then every day between 8:00-14:30. The charge of 45,- zloty for the blood test is payable at the institute.
Physical Examination
All vaccinations can be done at each office or at a nearby facility.

The charge of 50,- zloty is payable at the secretary's office, then the blood sample is taken in the Admissions Room.
2. Blood samples are also taken at Centrum Kardiologiczne (Cardiology Center) "Amicor" (where general medical check-ups are also done), Kraków, ul. Krakusow 4, tel. (48)(12) 623-45-45 or (48)(12) 623-45-55 between 9:00-11:00. The fees are: syphilis test - 20,- zloty, HIV infection test - 60,- zloty. Results are ready the same day at 14:00.

VACCINATION REQUIREMENTS
U.S. immigration law requires certain categories of visa applicants to obtain the vaccinations listed below prior to the issuance of a visa.
The vaccination report is not required for K - fiance(e)s and K3/K4 applicants. Immigrant visa applicants, however, must submit this report as part of the medical examination process.
Panel physicians who conduct medical examinations on behalf of visa applicants are required to verify that visa applicants meet the vaccination requirement, or that it is medically inappropriate for the visa applicant to receive one or more of the listed vaccinations: mumps, measles, rubella, polio, tetanus and diphtheria toxoids, pertussis, influenzae type B (HIB), hepatitis B, varicella, pneumococcal, and influenza.
To assist the panel physician, and to avoid processing delays, all visa applicants should have their vaccination records available for the physician’s review at the time of the medical examination. Visa applicants should consult with their regular health care provider to obtain a copy of their immunization record, if one is available. If you do not have a vaccination record, the panel physician will work with you to determine which vaccinations you may need to meet the requirement. Certain waivers of the vaccination requirement are available upon the recommendation of the panel physician.
Only a physician can determine which of the listed vaccinations are medically appropriate for you, given your age, medical history and current medical condition.
